The information sought in the RYLA application form is requested so that we can provide assistance in case of emergencies and to plan effectively to meet the needs of the person about whom the information is collected.

The answers that you give will be confidential. You are not obliged to give us this personal information. However, if you choose not to provide these personal details we may not be able to provide you with the full range of our services.
