Emergency Medical Information Booklets Community Project – 2005/2006

 

As a key service to our local community, particularly our aged residents, we introduced the Emergency Medical Information Booklets project this year. 

 

The Booklets printed in conjunction with Metropolitan Ambulance Service Victoria provide for the recording of important elements relating to the owner of the booklet.  The booklets are popped on the refrigerator in their clear plastic sleeve – it has magnets on the sleeve.

 

The aim of putting the booklet on the fridge is so that Ambulance Officers will easily find it.

 

The Booklets have provision for General Information such as Personal Details, Date of Birth, Pension No, Ambulance Cover No, Medicare No, Private Insurance details, Local Doctor, Next of Kin and Emergency Contact Numbers. 

 

Details of Medications are also able to be recorded including dosage and times.  Medical Conditions including allergies are recorded plus availability for tick a box for Heart Attack, Angina, Epilepsy, Diabetes, etc.  In the final section is an area for Patient History and key events of importance. 

 

The booklets are aimed at being completed by family / friends initially and with the assistance of medical practitioner / ambulance if needed.

 

The Rotary Club of Mont Albert & Surrey Hills distributes these booklets through seven local participating businesses:

Residents and customers are welcome to purchase the books for a gold coin donation.  The donations then assist in the purchase of new stock to continue the replenishment of the boxes at our outlets.
